Clinical Efficacy of Diagnostic Gynecologic laparoscopy

Abstract
The diagnostic laparoscopy remains a simple, relatively safe, and accurate procedure essential in the evaluation of many gynecologic problems, the purpose of this study was to assess the ability of diagnostic laparoscopy in identifying various
organic
gynecologic lesion. A total of 499 cases that underwent diagnostic laparoscopy for a variety of gynecologic indications from January, 1989 to July, 1993 were retrospectively reviewed.
@ES The results were as follows:
@EN 1. Clinical indications for diagnostic laparoscopy were suspicious ectopic pregnancy (27.1%), infertility(24.2%), tubal sterilization reversal(23.4%), pelvic pain(17.2%), obscure pelvic mass(5.2%), and primary amenorrhea(2.8%).
2. the abnormal laparoscopic findings were demonstrated in 84.4.% of infertility cases. they were represented bilateral tubal occlusions(27.3%), unilateral tubal occlusion (20.7), normal pelvic organs(14.9%), pelvic adhesion(14.0%), etc.
3. Comparison of laparoscopy in 86 cases of hysterosalpingogram disclosed that consistent findings between laparoscopy and hysterosalpingogram were 49 cases(60.0%).
4. In 135 cases of clinically suspicious ectopic pregnancy, 75 cases(57.8%) were confirmed by lapparoscpy, of which 67 cases(89.3%) were noted as unruptured.
5. In 32 cases of acute pelvic pain and 30 cases of chronic pelvic pain, the most common findings was ovarian bleeding (31.2%) in acute and pelvic pain and 30 cases of chronic pelvic pain, the most common findings was ovarian bleeding(31.2%) in
acute
and pelvic adhesion(36.7%) in chronic.
6. The 6 of 11 (55.5%), 5 of 9 cases(56.0%), 5 of 8 cases (62.5%) and 3 of 6(50.0%) cases were clinically suspected, respectively were confirmed by laparoscopy.
7. In 14 cases of primary amenorrhea, mayer-Rotiansky-Kuster-Hauser syndrome was 6 cases(42.9%).
8. The complication rate for all diagnostic laparoscopy was 0.4%.
It is considered that diagnostic laparoscopy is a valuable and safe procedure helpful in delineating a number of gynecologic disorders as well in determining their disease extent. It should, however, be kept in mind that diagnostic laparoscopy is
an
invasive procedure. whose use should be kept to a minimum to cases in which the benefit of the procedure clearly outweighs risk involved with it.
